It was developed by independent developer John Kolbek and Nate Sanders, a former QA tester at Rockstar Games. The game was released on Steam on May 22, 2015, as a free-to-play title, and the Xbox One, PlayStation 4, and Nintendo Switch versions of the game were released on June 26, 2017. The PlayStation 4 version of the game is backwards compatible with the PlayStation Vita.
The game features an art style based on European post-war horror films, including Night of the Living Dead. Players control a player character named Griffin who has lost his memory. Griffin is joined by a playable character named Sweet, a human girl with psychic abilities who was drawn into Griffin's "dreamscape". While playing the game, players guide Griffin to his goal by completing numerous and fun missions.
The gameplay of 123 Slaughter Me Street sees players explore a series of dreamscapes populated by various characters and enemies. The game has a set of goals for Griffin to accomplish within each dreamscape. Players use their psychic abilities to gain access to areas they cannot physically explore. As players explore the dreamscapes, they unlock additional abilities that help Griffin in his journey.
As players complete missions, they gain experience points that can be used to unlock more powerful abilities. The game contains a large number of collectible items to help Griffin along his journey. Each time Griffin completes a mission, players can continue on to a new dreamscape, and each dreamscape can have multiple goals.
123 Slaughter Me Street can be played cooperatively with a friend or purchased as a single-player game. A download code for the full version of the game was included in the special edition of the PlayStation 4 game The Unfinished Swan.

The game's soundtrack, composed by Jonathan Pinckard, was released alongside the game. In June 2017, the soundtrack received a Special Edition release, which added 8 new tracks. In addition to the new tracks, the Special Edition also added a set of bonus in-game unlockable weapons, and a unique boss fight.
